Community Health and Wellbeing Worker Project - Bringing Brazil to Selby!
Community Health and Wellbeing Workers play an essential role, supporting households to access health services and community support, helping to address rising inequalities in health and social care, and identifying unmet needs in the community.
- Each household has a named worker
- The service is provided to each person living in the household
- we are connected to your GP who will allocate your household to us
- The named worker will stay with you as long as you need them, and as long as you are registered to the GP practice
Each Community Health and Wellbeing Worker has a defined number of households and this project is currently working on streets in Selby West Ward (around Flaxley Road). Workers visit people in their homes to provide health and wellbeing advice and connect them to NHS and community-based services and activities.
Based on an existing health model in Brazil, this exciting project is being delivered by Selby Town Primary Care Network in partnership with Up for Yorkshire, funded by the Integrated Care Board.
What does a community health and wellbeing worker do?
- We tailor your care according to you
- We have the time to listen to your social, health and wellbeing needs and concerns without judgements
- We empower you to take control by giving you tools and resources
- We will inform you of current health offers such as screening and health checks
- We can even connect you to the community, advise you on any community activities and help you to start up your own groups, if this is something you’re interested in
